			<content:encoded><![CDATA[<p>A lot of women ask if they can breast feed after Breast Augmentation Surgery.  The answer is a resounding yes.  For the vast majority of women who have a BA breastfeeding is no more difficult with implants than without.  In fact, some women who have breast fed with and without implants say that breastfeeding with implants is easier!  </p>
<p> Breastfeeding is a growing concern with patients who have had Breast Augmentation surgery.  In previous years, women who received implants were married and had already finished with childbearing.  However, more and more single women, and women who have not finished or even begun childbearing are having the surgery.</p>
<p>In 1992 the first report of a Silicone Illness hit the media.  At that time there was fear that breastfeeding with silicone implants would endanger the child.  There has been studies performed to show this not to be the case.  The main reason being that the silicone molecule is too large to pass into the milk ducts.</p>
<p>Later, Silicone was removed from general use, and Saline implants were the only available devices on the market.  Even if the saline did leach into the milk, it is an inert substance, with no harmful effects on mother or baby.</p>
<p>Some concerns are placed on implant placement, and incision site.  It is said to be more optimal to have the implants placed under the muscle, and to avoid the peri-aerolar incision.  The reasons are simple, using those guidelines, there is less interference with the milk ducts which reside directly under the skin and in the tissue above the muscle of the breast.  However, as with everything in science, this is not guaranteed.  There are many women who have had placement of implant and incision in sub-optimal locations, and are still very successful with breastfeeding.</p>
<p>It is very important to discuss your plans of breastfeeding your baby at the time of your consultation.  Your surgeon will be able to work with you, to get the best possible results, even if you are not planning on having children anytime in the near future.  </p>
<p>Breastfeeding is still the preferred method of feeding a baby by the American Academy of Pediatrics.  </p>

<p>&#13;The first thing that has to be established is why the surgery is wanted. Any woman who is getting this surgery to make her man happy and not doing it for herself is making a mistake. The person that they are doing it for could be gone tomorrow. Unless they have another surgery, those breasts are going to be changed forever.</p>
<p>&#13;For a woman who has smaller breasts, frustration is sometimes experienced because they do not feel that they can wear sexy clothes. If having larger breasts will give them the confidence and allow them to wear the clothing that they want, why not get the surgery? Having surgery for a reason like this can be a huge boost to any woman&#8217;s confidence.</p>
<p>&#13;Low self esteem is often used as a reason for someone to get breast surgery and this is very tricky ground. If they do not get the desired results in public opinion that they were expecting, they can often be damaged further. This will lead to a downward spiral that they may never recover from.</p>
<p>&#13;It is common practice for a surgeon to require patients to go through a quick psychiatric questionnaire. This is to make sure that the patient is doing the surgery for the right reasons and not something that could lead to mental issues down the road. Once they are satisfied that the breast surgery impact on the psyche will not be negative, they will move forward and the patient will hopefully see the breasts that they have always dreamed of.</p>

The best candidates for laser bra breast surgery are well-adjusted women whose sagging breasts cause health and medical problems, interfere with routine activities, disrupt sleep, or suffer related self-consciousness. The procedure may work well for candidates who are concerned about nipple sensitivity and breast feeding after surgery. Patients with certain medical problems may be excluded from this treatment: for example, older patients with a heart condition on many medications would significantly increase the risk of complications.</p>
<p>&#13;<br />
A small percentage of plastic surgeons today have experience with the laser bra breast procedure. It is important to seek the advice of an experienced, qualified laser bra breast surgeon to make a final decision about whether the procedure is right for you.</p>

The Laser Bra Procedure</p>

The location of the procedure is typically a hospital or outpatient surgery center. Time in surgery is up to two hours. The procedure is most often performed under general anesthesia, while you are asleep, and patients typically return home within two hours after the procedure.</p>

Recovery from this procedure is typically five to seven days. You can resume normal activities within three weeks. The doctor will supply a complete list of instructions that must be followed to reduce the risk of complication. You may be fitted with a medical garment to wear at all times, other than bathing (sponge bath), until your doctor determines it is no longer needed.</p>

<p>Some of the most common revisionary breast surgery are because of deflation of a breast implant, capsular contracture formed around one or both implants, double bubble formation, etc.  Below you will learn about the most common revisionary breast surgeries performed and what to expect if you become a candidate for this surgery.</p>

<p><strong>Symmastia</strong></p>
<p> </p>
<p>One of the more common problems that patients are seeking Dr. Pousti&#8217;s help for is symmastia.  Symmastia more commonly referred to as bread-loafing or uni-boob, occurs when two implants touch one another in the center of the chest. If the horizontal muscle that is connected to the sternum and goes across the implant is cut during surgery, then the implant can move toward the middle of the chest.  A possible consequence of unopposed muscle action with overzealous medial release causing aesthetic disfigurement and additional issues with discomfort and apparel problems, symmastia is a difficult surgical complication to address.</p>
<p> </p>
<p>Who is Susceptible to Developing Symmastia?</p>
<p> </p>
<p>            Patients with multiple breast operations, excessively large implants or overaggressive medial dissection are susceptible to developing symmastia.  Symmastia seems to be more prevalent among thin women mainly due to the fact that thin women usually have less tissue or fat covering the sternum.  Also, patients with pectus excavatum, a depressed breastbone, are more prone to symmastia.  Attempts to increase cleavage by releasing the soft tissues or inner origins of the pectoralis muscles lead to symmastia by surgically interrupting the natural barriers of tissue and muscle at the medial location of the breasts where the cleavage is normally defined. </p>
<p> </p>
<p>How is Symmastia Treated?</p>
<p> </p>
<p>            Symmastia is difficult to treat and recurrence is common.  Correction requires combined restoration of the initial presternal subcutaneous integrity and medial closure of the pocket.  In order to repair symmastia, the space between the 2 implants must be securely closed off.  The combination of medial closure of the breast implant pockets and suturing of the preseternal soft tissue to the sternum periosteum provides one satisfactory option to the surgical reconstruction of symmastia.  Often, it is necessary to “open” the breast implant pocket laterally (to the sides) to decrease pressure along the cleavage area.  There are several methods used to correct the deformity, such as allogenic dermal grafting, fibrin-based tissue glue, and delayed filling of an adjustable implant.</p>
<p> </p>
<p>What to Expect after Revisionary Surgery?</p>
<p> </p>
<p>            After corrective surgery, the patient is instructed to wear a t-back sports bra or “thong bra” for at least two months.  This brassiere is utilized to help apply pressure and assists in keeping the area stable allowing the area to heal and aiding in recovery.  After this specific surgery, patients are extremely careful to allow for full recovery.  Dr. Pousti is careful to follow up with patients and remain close by their side to ensure a full recovery.</p>

<p><strong>Implant Mal-Position</strong></p>
<p> </p>
<p>One of the most common complications after breast augmentation surgery is implant mal-position. This occurs when the implant is in an incorrect position on the chest wall.  This may include incorrect position of the implant superiorly (“riding high”), inferiorly (“bottoming out”), medially (“symmastia or uni-boob”) or laterally (falling outward into the axilla). Dr. Pousti, at Pousti Plastic Surgery, has corrected these problems for many patients.</p>

<p>Bottoming Out</p>
<p> </p>
<p>Bottoming out involves inferior migration of the implants.  This causes the nipple areola complex to appear too high on the breasts.  Also, the distance from the areola to the inframammary fold is too great.  This is corrected by “raising” the inframammary fold using internal sutures.  This is done after careful measurements are made from the areola to the “new” inframammary fold.</p>

<p>Tuberous Breasts</p>
<p> </p>
<p>Tuberous Breasts have a very narrow base and usually a long skin envelope. Sometimes people refer to &#8220;Tuberous Breast&#8221; shape as the shape similar to &#8220;snoopy&#8217;s nose&#8221;. In the most severe cases of Tuberous Breast, a breast lift can be done through an incision around the areola, making the breast into a more rounded shape that the patient will be happy with. In many cases however, a lift is not necessary. The patient should be aware that the final result will take months to see and that they will need to be patient.</p>

<p>Lateral Displacement</p>
<p> </p>
<p>Some post operative breast augmentation patients present with<strong> </strong>lateral (outwardly) displacement of the breast implants.  This can be corrected using an internal suture technique decreasing the size of the pockets and moving the implants toward the midline.</p>

<p><strong>New Inframammary Fold</strong></p>
<p> </p>
<p>Some patients present with a short distance from the areola to their inframammary fold. Many of these patients wish to increase this distance and the “fullness” of the lower pole of the breasts. This distance can be increased in the operating room and the fullness of the lower pole of the breasts enhanced with the implants. A major portion of Dr. Pousti&#8217;s practice involves helping patients who wish to enhance the appearance of their breasts (breast augmentation). Some patients present with the finding of a short distance from their areola to their inframammary fold. The patients often wish to increase that distance and the appearance of &#8220;fullness&#8221; of the lower pole of the breasts.  Careful measurements area made in order to achieve as much symmetry as possible – care is taken to avoid over-dissection and a resulting “bottoming out” of the implants.</p>

I am speaking to a lady who has not only had breast cosmetic surgery once but over 5 times. Well the first question in my mind to ask is why? Why would you want to put yourself through pain, she happily replied back to me, well she said for the money and most of all I love the attention, with bigger breasts I can get any job I want.</p>

Ok I stated to myself secretly thinking to myself if youre not very clever I wouldnt employ you if they were the size of 10 footballs. My second question I threw to her (keep in mind I only have 3 questions to ask). Did it hurt? She stated I dont mind the pain as I know Im going to get something great out of it, plus the surgeons I choose do it very cheap. Ok thats fine I thought to myself.</p>

My final question had to be a great one was struggling to think about it but all I said was, What sort of attention do you get?, with a little giggle at the end. Well she said when I walk into a room everyones head turns, when I go for a job I always get it and best of all when I go out I get free drinks and meals.</p>

So the conversation is over and my conclusion is as follows, are women just getting breast surgery due them thinking to themselves that they will earn more respect in life, or are they just subconsciously thinking to themselves that people are respecting them more, lets head back to the interview questions and answers, firstly why would someone with larger breasts get a job? If it was me the only way I would give a person with large breasts a job is if they were clever or for pity, never from respect.</p>

The answer to the final question shocked me the most, she was stating everyones head turns, free drinks and meals well Im wondering to myself why there heads turn yet again she feels its from respect but I feel people are just laughing at her.</p>

All in all if you are looking to go and get breast reduction or enlargement done, do not just go to any butcher as you may pay for the rest of your life. Find a good company with a great reputation, things will never go wrong, and never choose to have surgery to gain respect.</p>

Strolling to the window to see if she has gone, I notice a big crowd around her I am thinking what the hell is going on so I run around and get my binoculars everyone is jumping around with pens, Oh my god I thought to myself that wasnt Pamela Anderson I just spoke to was it? Then I looked around abit further and someone had picked up her breast implant with a pen, so thats what they were trying to do. Looks like she should take my advice and go and get a professional surgeon, because she will definitely need one now.</p>
